Ceph

Allow starting a mon with an empty monmap and "mon hosts". Make it
take "peer hints" via the admin socket. Peer hints are a list of
ip:port, and are added to the mon's existing knowledge of peers (add
not replace).

This combined with #2418 should let us bring up dynamic monitor clusters from scratch.
